This only seems to occur when channels are being made and released at a high pace . Was not able to simulate the condition but most of the times asterisk recovers if the channel in question is released through a softhangup it seems . Recovery in 24 minutes were huge would have crashed under load in such a case could you confirm if the same happened in your case and recovery happened when PJSIP/sip-3-0002949d was hangup from logs .
