Aditotnal information which might help: when chan_sip is used instead of chan_pjsip on the same machine: no such behaviour occurs. Please let me know if you would like to see sip.conf as well.

Randomly the PJSIP will reply 481 Call/Transaction Does Not Exist to the trunk's CANCEL request of ongoing call in Ringing state. And sometimes (for no apparent reason) it just processes CANCEL request as expected.
