In addition please provide any Asterisk log that was captured in the minutes leading up to the crash.

Preferably if you can reproduce the issue then gather a debug log as per the [instructions on the wiki|https://wiki.asterisk.org/wiki/display/AST/Collecting+Debug+Information].
