You need to provide us with a method to reproduce the issue, or provide us with enough debug to identify the issue simply.
It doesn't appear that you know how to reproduce the issue at will so you'll need to provide us additional debug.
You didn't provide the debug logs requested (instructions at the link in my last comment). Perhaps they are unavailable if you are unable to reproduce the issue.
To make any progress with the issue we'll at least need a packet capture showing all the IAX network traffic at the time of the issue and a debug log (again as described in the instructions) captured at the same time.
