Please attach gather a log following these instructions: https://wiki.asterisk.org/wiki/display/AST/Collecting+Debug+Information 

and attach it to the issue as .txt file. 

The log should include the DEBUG and VERBOSE logger channels turned up to the level specified in the instructions.
