I've attached the requested files.
Scenario test003 calls extension 7
test002 pickup the phone holds and make call to extension 4
test001 pickups the phone
test002 make AT
test001 hears MOH, test003 hears test001
What I have found (it may be a clue):
1) it only happens if I have macro configured
2) I could not reproduce the problem if test002 was a Linksys phone. If it was a gigaset or grandstream the bug occurs. (Linksys as yealink did not send last invite with hold to the caller before sending Reffer).
