The [Wiki for REF_DEBUG|https://wiki.asterisk.org/wiki/display/AST/Reference+Count+Debugging] has some outdated/incorrect information.

# We should recommend updating to the versions which include the menuselect option.
# Information about enabling REF_DEBUG for specific modules should be removed.  This is no longer supported and never worked well anyhow.
# Uploading full refs log is not possible, it almost always exceeds JIRA attachment limit.  We should recommend that people run refcounter.py with the '-n' flag.
# Example refs file needs updating.
# Information about old refcounter utility should be removed, replaced with information about refcounter.py

I realize that the old information applies to older versions of Asterisk.  Any bug that could be triaged by a refs log needs to be reproducible on the current version of that branch, otherwise there is a decent chance it's been fixed.  The [Asterisk Issue Guidelines|https://wiki.asterisk.org/wiki/display/AST/Asterisk+Issue+Guidelines] page clearly says to update to latest in branch.

If needed I can make the changes myself, I'm posting here to run it by other bug marshals to accept objections.  I feel these changes will make it easier to request more information from users.
