David, thanks for the additional information. However, I don't see DEBUG message types or AGI debug in your full log. Is there a reason you were unable to get those?

You might also try to get "agi set debug on" output for the next crash. Make sure that is captured along with a full log file where DEBUG and VERBOSE levels are turned up to 5 or higher. See the wiki instructions
