Can you go ahead and attach your full queues.conf and queuerules.conf? There may be some combination of settings that is leading up to this, and I'd like to make sure that we can reproduce it using the correct configuration. It would also be good to have a DEBUG log leading up to the crash, as somehow the queue penalty rules got blown out. Did a reload operation occur?
