This is definitely a regression in SVN.
Since I was able to reproduce easily with some CLI commands I tested some older revisions.
The last stable Asterisk revision was 399516 which we installed on September 27th 2013.
The only commit to chan_iax2 since that date is:
Revision 401016 - Directory Listing

chan_iax2: Fix channel left locked in off nominal code path.
So this IMHO must be the cause of the regression.
