[2013-11-18 10:44:41] ERROR[28189]: chan_iax2.c:2470 peercnt_add: maxcallnumber limit of 2048 for 192.168.192.140 has been reached!
That extension is a Digium IAXy.

And then 'iax2 show peers' displays nothing (locked).

iax2 reload does not recover the module, reload doesn't complete.

Restarting Asterisk brings back my iax2 peers.
