Finish up the implementation of picking up a parked call. This should:
* Take a channel out of the wait bridge
* Create a new two-party bridge
* Place the channel performing the pick up into the two-party bridge along with the channel that was in the wait bridge.

Note that this should also include making sure that the Call-ID associated with the various channels is merged appropriately.
