If I tested with an interval and duration under spec than I could get a similar CPU usage issue to happen immediately even with a single call.

I couldn't reproduce it with an interval and duration within spec unless I set rtpkeepalive. Without it set I couldn't reproduce it even while streaming DTMF constantly on every channel at the interval and duration you specified with over 30+ simultaneous calls for over a minute. Even at that CPU usage would barely hit %10, sometimes up to %15.  

With rtpkeepalive on I always got the CPU usage spike at least 15-20 seconds into the call, regardless of how many calls were running. 

I don't have any further time to dedicate to triaging this single issue. At least there are two possible ways to reproduce it documented here and the issue is open for developers to look at. Thanks for the report.
