Please provide the full Asterisk debug log covering all calls in the PCAP with VERBOSE, DEBUG messages enabled at level 5.

Additionally provide any configuration files relevant to the channel drivers and other resources in use.. 

Ideally you should be able to describe exactly how to reproduce the issue including the configuration of every file needed. Thanks!
