Jeff,

For the "stuck in queue debug" , can you get the same debug but with VERBOSE at level 5 enabled as well? 

Other items that will help anyone looking into this:

-point out where the stuck call begins just to make it easy on those looking through it.
-a SIP & RTP packet trace on both systems for the first call that gets stuck would be very helpful.
-attach sanitized queues.conf configuration and sip.conf configuration.
