for the conversation to move over to the new IP you SIP client
must send a reinvite to asterisk with the to & from tag from the curent conversation
the new invite must contain updatet ip address for both the SIP and the RTP
I know this is supportet in asterisk 10
as we use this in openBTS to make it posible to move the mobile phone conversation from one base-station to the next
