Summary: First DAHDI line transferred into ConfBridge hung up
when second DAHDI line transferred into conference
Description:
(1) Receive DAHDI call on Polycom phone using Dial(t). (Not sure if make of
phone is important.)
(2) Transfer that call using transfer key (not Asterisk codes) to
extension that has ConfBridge(1s). This is an attended transfer.
So far, no problem.
(3) Receive second DAHDI call as with https://issues.asterisk.org/view.php?id=1.
(4) Transfer that call to conference extension just as with
https://issues.asterisk.org/view.php?id=2.
After receiving the second call (step 3), hitting the tranfer key, and
calling the conference extension (step 4, which calls ConfBridge()), the
Polycom phone is in the conference with the first call. (Remember that the
transfer is attended.) But, when the Polycom phone hangs up to complete
the transfer, the first call is hung up.

I guess this part (near the bottom of the file after the second transfer)
looks suspicious.... although maybe it's normal:
