Summary:                    Deadlock using blind transfer
Description: 
Sometimes I get a deadlock  when we use blind transfer. The deadlock
freezes asterisk until kill -9.

A lot of transfer issues have been fixed recently. Please reproduce with
the latest 1.6.2 branch directly rather than 1.6.2.16.
