Summary:                    [patch] OOH323 Incoming and Outgoing Calls Fail
Avaya with Asterisk 1.8.1.1
Description: 
1. Upgraded to Asterisk 1.8.1.1 from Asterisk 1.6.2.15.

2. All outgoing calls started to fail.

3. "ooh323 show" was not recognized as a valid command. Auto-complete
worked though.

4 Rebooted the system.

Respective patch uploaded (issue18542-4). 
Difference to previous patch are send tcs in ooAcceptCall (start tcs after
call answered by us), move send tcs from ooCreateH245Connection to
ooHandleStartH245FacilityMessage (we start tcs after getting signal to
start only not in all cases of creating h245 connection).

sending of StartH245 message logic wasn't done there (we send this
messages regard on tunneling setting not fast start, it's incorrect in
common but insignificant in our tests for now) and i'll done this in
closing patch.
