Summary: [patch] appdocsxml.dtd updates and general xmldoc
updates
Description:
appdocsxml.dtd "ref" does not have "manager". Adding see-also
relationships causes the whole documentation system to fail loading if a
value is set that is not in the DTD.

Tested appsdocsxml.dtd.diff and meetme.c.diff on a clean system. Works
fine. The see-also section shows the relationships the same, so you don't
know if it is an application, function, or manager relationship. I will
think about this.
