dahdi-tools: 2.4.0
Asterisk: 1.6.2.13
Digium FAX Driver: 1.6.2.0_1.2.1 (optimized for i686_32)

I have been having issues with these two errors appearing when receiving
faxes from two of our branch offices. Both have different type of fax
machines, and both are capable of faxing to other faxes without issue. I
also receive faxes from most other locations without issue.
