The problem can't be reproduced, it's random.

Asterisk version is 1.6.0.14

You're using a pretty old version of 1.6.0.  It is very likely that this is
fixed in a later release.  However, if it's not, you will need to upgrade
to 1.6.2 before we can look at it.
