Description: 
This patch allows you to significantly increase the compatibility with
other equipment. There are two features:
- Opening voice channel on FastStartAcknowledged before Answer.
- Remove H245inSetupOptions for better capability. Because not all
equipment support it.

This is against code that doesn't have many (any?) active maintainers.  You
might want to try chan_ooh323 and see if you can get the same result there,
or create a patch on that code.  This should remain open, but honestly the
chances of someone with commit access picking it up and working on it are
slim unless you want to maintain the channel.  :-)
