Summary:                    AMI Originate does not return a "Reason" when called
with Async = 0
Description: 
For some reason, AMI Originate does not return a "Reason" when called with
Async = 0.  This makes it extremely hard to find out the causes of
potential failures (busy, invalid, etc.) and take the appropriate steps.
