Hi there...
I'm a colleague of tbelder and see something interesting;

This bug seems to be occurring in combination with the use of Local
Channels.
In our case we dial a Local channel and from that Local channel dial
multiple other Local Channels. When one of those Local channels is answered
we execute a new Dialplan (through the use of the dial option "M()").
Directly after this part of the dialplan is executed the asterisk displays
the "Fixup Failed" error and becomes unstable.

Can anyone confirm any similarities with our dialplan setup?
This might help in trying to find the reason why this error is
occurring...
