We need a manager event to be sent when a call is being forwarded.
Patch attached.
NOTE: I filed this under Applications/app_dial, but the patch also applies
to apps/app_queue.c

Wile nobody else tested the patch, I took a fresh copy of the trunk and
tested the patch again with a positive result.
