This is an incremental cleanup of transmit_* functions to:
 - better align with skinny protocol
 - remove building and sending frames inline
 - improve readability of the code

Added skinny-clean01.diff. Breaks transmit_tone into transmit_start_tone
and transmit_stop_tone as per the skinny protocol.
