Summary:                    Music on hold broken in 1.6.2.2
Description: 
After my last update to Asterisk 1.6.2.2, I cannot hear MusicOnHold anymore
when joining a queue.

I have also tried the following 

exten => 8484,1,MusicOnHold()

and I do not hear anything despite Asterisk showing

Executing [8484 at default:1] MusicOnHold("SIP/220-00000007", "") in new
stack
    -- Started music on hold, class 'default', on SIP/220-00000007

I can hear music on hold when I make a transfer or put call on hold
