Summary: Transfer = Yes not working
Description:
I have two Asterisk 1.4.37 server. Server A receive a dahdi call and
forward the call directly to server B which ring the receptionnist phone 1.
The receptionnist then transfer the call to phone 2 which is registered on
server A using the same trunk. When phone 2 answer, the iax2 channel is
never released.

I tested with 1.4.37,1.6.2.14 and 1.8.0 and the latest 1.8 revision

We'll need to see the dialplan as well because it's possible something is
configured or being used that doesn't allow the call to be transferred
since Asterisk needs to sit in the media path for the call.
