Description: 
The callerid is missing in ${CALLERID(num)}
This was working until rev. 215523 of asterisk branch 1.6.2
I went back to rev. 215467 of chan_sip.

Can you test the patch on https://issues.asterisk.org/view.php?id=15839 ?
I know that this bug was reported before that one, but it's the same issue
and that one has a patch.
Going to close this one and track progress on 15839.
