Description: 
Reopening this as new. Attended SIP transfer from Polycom UA, no queue
involved.

I have not seen this crash again - now running on about 10 machines for
multiple weeks. Previously, I couldn't get through a couple of days. It did
seem to bring https://issues.asterisk.org/view.php?id=15609 to the surface, but
I'll post notes in that bug.
