I have exactly the same problem.
The version I am trying to get running now is "Asterisk
SVN-branch-1.6.0-r193121" although I reproduced it on 1.6.0.9 as well.

DAHDI Version: 2.1.0.4

Thanks Tzafrir. Sorry, I had the lines disconnected at that point. It now
presents as "in use" with no alarm.

Span 1: WCTDM/4 "Wildcard TDM400P REV I Board 5" (MASTER)

I should have mentioned that incoming calls are handled fine.
