Crash, it appears, on random occasions when playing mp3 files in music on
hold (I believe based on the backtrace).

This looks like a duplicate of issue 0014045, which is a duplicate of issue
0014395.  Try upgrading file.c to revision 175407 and report back whether
or not it resolves your problem.
