Description:
bt attached - unsure how to interpret this one; this is the only time I've
seen this type of crash.

Given that this died in libc's malloc, I'm going to have to suggest that
you run under valgrind. However, if you're unable to reproduce the crash,
it may be exceedingly difficult to replicate the exact situation and get
the necessary debugging information.
