Summary:                    [patch] astcanary does not exit when asterisk dies
Description: 
In 1.6.x versions and trunk in case of segfault or when killed asterisk
can't stop astcanary, but start a new one on the next start, which may lead
to several astcanary running at the same time.

Cause astcanary to exit if Asterisk exits abnormally and doesn't kill
astcanary.
Also, add some documentation supporting the use of astcanary.
