Summary:                    Wrong order of Channeltype and Uniqueid sent to

Description: 
Typo exists in all asterisk 1.6 branches.
Information sent to manager is in the wrong order.
Channeltype and Uniqueid got mixed up.

Attached fix.

I think this could be committed right away.
