with integrated Neon Lamp

Using /etc/dahdi/chan_dahdi.conf keyword mwisendtype=hvac

This applies cleanly to latest trunk as of last night.
Prevent polarity reversals if neon is configured when sending FSK MWI and
when going offhook.

wctdm_ioctl DAHDI_SETPOLARITY exit if not FXS before getting user data.
