Summary:                    [patch] SIP Realtime SQL Table
Description: 
Since I have not been able to locate a properly documented MySQL table for
SIP Realtime, I have decided create one from channel_sip.c source.  Please
have a look at the attached file and comment on its structure.

Version 2: Updated more default settings, re-include some missing tables.

I am also curious if we should check to see if there fields exist in the
database using ast_realtime_require_field() function, we currently check
some fields when the module loads.
