Summary:                    Incoming Calling Rules has javascript errors
rendering forms useless.
Description: 
Using Asterisk 1.6.0.6 and Asterisk GUI 2.0.4, the "Incoming Calling Rules"
menu doesn't work.

Clicking it gives a javascript error in pbx2.js (in the getName & getType
functions).  The "new rule" form doesn't show any values because of the
previous error.  The trunk types array length is 6 just before the crash
line but the trunk names are NULL.
