Sorry putnopvut I'd love to help but I can't reintroduce the crash into
this customer's environment.
Jvandal's solution has prevented any segfaults from happening.
The workaround does fix my reported problem.

I think jvandal also implemented something else into dialplan to fix an
outgoing call problem related to this dial plan...

dialplan...

exten => s,n,GotoIf($["foo${FAXMODEM}" = "foo"]?$[${PRIORITY} + 1] :
$[${PRIORITY} + 2])
exten => s,n,MixMonitor
