This patch adds skinny support to res_phoneprov

NOTE:
the serverport configuration setting is now sipserverport and a new
setting skinnyserverport is added.
In the templates you should now use SIPSERVER_PORT and SKINNY_SERVERPORT

Suspended for now until I (or someone else) comes up with a patch that also
implements tftp because the skinny phones cannot be provisioned over http
