Description: 
I am opening a new feature request, with patch, because the resolution to
13497 only partially fixes the problem described therein, and I believe has
bugs if the user attempts to rename a trunk.

The attached patch provides far more flexibility. This makes changes to
tooltip.js, pbx,js and trunks_voip.html.

tooltip.js... Some additional help for the add VoIP Trunk dialog

pbx.js... logic to determine what to base the context name on.

trunks_voip.html... new field in the add VoIP Trunk dialog. Tests for
duplicate context names. Disables ability to edit provider name or user
name while editing existing trunk if it appears that the context is based
on this. If user wishes to change a disabled field then they must delete
then create new trunk.

Please consider this for inclusion in asterisk-gui.
