Summary:                    ExternalIVR will hang a channel attempting to read
from a file descriptor that has somehow gone away
Description: 
ExternalIVR checks for an exception and that the file descriptor isn't set
to an EOF, not whether the file descriptor is actually there.  These
patches check that.
