Summary: Migrating stdexten and stdPrivacyexten in
extensions.conf from Macro to Gosub
Description:
The Macro() application is being deprecated.

So the stdexten and stdPrivacyexten Macros which the default
configs/extensions.conf.sample file contains should be updated to use Gosub
and arguments instead of Macro.

The examples in this file that use it have also been updated.
