The latest logs are from the system using 1.4.20.1 . I've upgraded it to
1.4.21.1 too and will send if the problem occurs.By the way disabling IAX
moule significantly decreases the problem occurance. 

 We have a patch including all fixes not included in distributions. Maybe
it's causing the line numbers mismatch. I can send you the patch applied
sources.
