Summary:                    [patch] MFC/R2 support for chan_zap
Description: 
Here we go. This is my first try to give R2 support to chan_zap. I'm sure I
am missing locks and/or features here and there but I have tested it
internally with success with a considerable amount of concurrent channels
(64). That's the best I can do with the hardware I currently have (more
coming!).

Hi Moy, thanks a lot, you rock.

The telco couldn't tell me for sure how long to block the line so I'll
have to figure it out experimentally next time I have broken lines.

It seems I don't get broken lines every time I have a timeout. I had 13 MF
back timeouts yesterday, all of them on incoming calls, and no broken
lines.

I'll bump my timeout up to 2000ms (but it's close to that now, default
setting, 1500ms).
