Zaptel don't send DTMF tones correctly on FXO lines. For instance when
trying to dial out a number.

OK, here is the complete trace but as I said trace is not meaningful at all
because if you would compare a working call with a not working one the
trace is exactly the same.
The bug is silent from a trace point of view!

The number is being dialed from zaptel at 19:52:45 exactly when on hook
transition complete event arrives and I think EC is out of matter in this
issue (I only talked about it to explain the trace).

As I said my suspicion goes either to _do_dtmf that can't get the dtmf
tones (NULL returned by zt_tone_dtmf? or the generated dtmf tables are
simply not dtmf compliant).
