Description: 
Zaptel don't send DTMF tones correctly on FXO lines. For instance when
trying to dial out a number.

I guess:
- either DTMF tones are now badly generated in such a way that FXO lines
can't understand their meaning
- Or the __do_dtmf method is simply failing to do its job (zt_tone_dtmf
maybe?)

But that's actually only a clue to look for...

Did you make any test while testing the new branch? Are dynamically
generated tables the same as their compile-time equivalent?
