Same issue here, running 1.4.11 and zaptel 1.4.6
Seeing anywhere from 12 to 50 percent of calls go into Cause 34 status
when auto-dialing beyond 12 lines at a time.   I tried putting waits in the
dialplan to space out the requests for a channel, thinking that might have
some effect, but I never got consistent results from my testing.
