Zaptel don't send DTMF tones correctly on FXO lines. For instance when
trying to dial out a number.

Unfortunately, I no longer have spare cards with which to conduct testing;
however, given the above comments I would like to note the following:

* In all cases I was using a version of ztcfg and fxotune that matched the
wctdm and zaptel kernel modules.  The build was conducted as follows
(except from Arch Linux PKGBUILD script):

# ./configure --prefix=/usr --sysconfdir=/etc --exec-prefix=/usr
# make VERBOSE=1
# make DESTDIR=$startdir/pkg install-modules
# make DESTDIR=$startdir/pkg install-programs

* I could not audibly discern the difference between DTMF that was
recognised by the telco switch and DTMF that was not recognised. 
Therefore, aural testing will not be sufficient.

* Earlier comments state that looping and FXO to and FXS port did not
reproduce the fault.  The fault was only evident against the actual PSTN
line.  This suggest that either the DTMF generation and detection code are
related (I am not familiar with the code base) or that Aterisk is able to
interpret the broken DTMF generated.  (Could this also be related to line
length and frequency attenuation if the generated DTMF is borderline?)  On
this basis looping and FXO port to and FXS port in order to debug and
resolve this issue will not be successful.

In summary:

* The fault is not related to ztcfg/module version mismatch (although this
may potentially generate the same or a similar symptom).
* The fault is not detectable by ear.
* The fault is not detectable via FXS/FXO loopback.
